Chronic conditions requiring medications: If there is a chronic medical condition that requires you to take ongoing medication, please provide a letter from your prescribing physician that states the following:

• Your condition is stable on the current medication

• There have been no recent changes on the medication dosage

• You have the ability to safely operate a commercial vehicle while on the medication.

Anti-anxiety or depression medication require a letter indicating you are on a stable dose and free of side effects.
