Prior to your appointment, we kindly request that you print these documents, complete them, and bring them with you to your first appointment at Comprehensive Pain Centers.

Because we have many requests for medication refills, we ask that you give us 5 days NOTICE for all prescription medications you need to have refilled.
Please take note to when your prescription medication runs low and call our office if you need a refill.
When calling our office for a refill (leaving a message) you will need the following information:
• Patient's name (please spell name)
• Name of the medication
• Dosage prescribed
• Date of last refill
• Your Pharmacy telephone number
